Terms of Service
Initial Meeting
New phone enquiries of 15mins are complimentary. New clients will require a 1 hour (minimum) face to face Initial Meeting. This can be conducted as a home visit or via Telehealth.
During the Initial Meeting we will clarify your goals and needs, and subsequent contact will be determined by mutually agreed upon Occupational Therapy / Care Coordination plan.
Consent
All new clients (or their legal representative i.e. power of attorney) will be asked to provide written consent for the Occupational Therapy clinician to act on your behalf and share information about your care. This will be discussed and requested at the Initial Meeting. Where you have nominated a substitute decision maker to act on your behalf, this nominated person will be asked to provide written consent.
Payment of Fees
1) You agree to pay the service fees as quoted in writing by your clinician.
2) Service fees will be invoiced to you at the completion of the service.
3) Where you have nominated another person to pay your service fees, this person will receive a Tax Invoice on your behalf.
4) Payment terms are strictly seven days from the date on the invoice.
Cancellation
Please advise your clinician if you are unable to attend an appointment due to illness
or carer unavailability with sufficient notice (24hrs). We understand you may need to
reschedule an appointment on occasion. As a small business, we request you provide as much notice as possible.
We kindly request written cancellation via email admin@holisticcarecoordination.com.au or phone call directly with the clinician on 0432 409 788. We will do our best to reschedule your appointment at a mutually convenient time.
A cancellation fee of $180 per hour will be applied to:
1) Appointments not attended as scheduled by the client without the required notice (as detailed above).
2) Last minute cancellation - less than 24hrs notice.

Travel
Private Clients and Medicare Clients:
Travel fees apply where you have requested a Home Visit by your clinician and you reside more than 10km from Geelong CBD.
Travel fees are based on the ATO ‘cents per km 2024-2025 rate’ of $0.88 per business related km, for clinicians to travel to and from your home address.
Travel fees apply to clients on the Bellarine Peninsula, Surf Coast, Barwon Heads,
Queenscliff, Bannockburn, South-West Victoria and greater Melbourne.
You have the option to request a Telehealth appointment to minimise travel fees.

NDIS Clients:
NDIS related travel costs (clinician travel time and distance / km) are set by the NDIS
Pricing Arrangements and Price Limits Guidelines 2024-25.
